The following documentation is required on your first appointment:
- Photo identification
- Insurance card(s)
- Doctor’s referral/Rx
- Completed New Patient Paperwork
- Specific Completed Functional Scale(s), only if requested when making your appointment
- Please wear loose fitting clothing, as this makes it easier for your therapist to treat you.
Your evaluation appointment will last approximately forty-five to sixty minutes, depending on the program built to fit your needs.
